The stimulus to write this book originated from the students with whom author had been associated during 25 years of teaching in six dental schools. In the innumerable continuing education courses that author had given over the span of those years. He had been able to see many of his former students, and invariably, these practitioners had encouraged him to formalize his teachings into a written text. In indeed, his educational efforts had contributed to the success of his former students, then it is his sincere hope that this textbook will serve to help may others with whom he hereby share whatever knowledge he possess.
